The sequence ends with Isaac feeling nothing, and a blank piece of paper. The memories change though to happier memories, Isaac playing with his cat, being tucked into bed and, towards the end, seeing his parents happy at Isaac’s birth. You find Dad’s Note and start retracing your steps back up until you reach Basement 1. However, the newest ending involves turning around and going back. Alternatively, you defeat Mom then descend downwards to kill Hush, then Delirium. Then you either descend into Sheol or rise up into the Cathedral, before fighting the Lamb in an inky abyss or fighting yourself in the Chest. Once you defeat Mom, you go into her body and kill her heart. The further you go, the more messed up things become. Most endings in the Binding of Isaac involve going deeper and deeper into… well, somewhere. So that’s how I’ve seen most of the new endings. That being said, a big chunk of the streamers I watch tend to play Binding of Isaac, so I’ve been watching their content as well, where they’ve all been playing the new DLC. And I’ve barely managed to do anything in the base Binding of Isaac: Rebirth game. That’s probably for the best because the Repentance DLC REALLY ramps up the difficulty. Okay, with that out of the way, I want to mention something first. Please don’t actually read any further if you don’t want to be spoiled. But the fan-made Antibirth mod was such a hit that the developers had to tie it all together for one final DLC. Sure, the developers said that about Afterbirth+. Shown throughout the game at loading points between floors is Isaac recollecting about his childhood while curled up in a fetal position and crying. He finds a trapdoor under the rug, and just as his mother bursts in, he opens the trapdoor and jumps down it. The game then transitions to the title screen.

Isaac is watching through a sizable crack in his door and starts to panic. She obliges, grabbing a kitchen knife and goes to Isaac's room. It states she has done well but says it still questions her faith and commands she must sacrifice Isaac. Once again, she follows its command and locks Isaac in his room. The voice again calls out to Isaac's mother, stating Isaac must be isolated from all the evil in the world. She obliges, taking away his toys, pictures, his handheld video game console and even the clothes on his back.
It asks her to remove all sin from Isaac to save him. Isaac's mother then hears "a voice from above", stating her son is corrupted with sin and must be saved.
Isaac (a kid) and his mother live in a small home situated on a hill, both peacefully keeping to themselves, with Isaac drawing doodles and playing with his toys while his mother watches Christian broadcasts on the television.
